The MEASURE Workflow
A systematic measurement workflow ensures accuracy, reproducibility, and consistency across every ultrasound examination. The sequence below — remembered by the mnemonic MEASURE — guides the sonographer from machine setup through final report generation. Each step builds on the previous one, so skipping or reordering compromises the quality of the measurement.
M–E–A–S–U–R–E

Select the appropriate transducer (frequency & footprint), set the organ-specific preset, and verify machine calibration.
Curvilinear for abdomen, linear for superficial structures, endocavitary for TVS. Confirm thermal indices (TIB/TIS) within safe limits.

Ergonomics & Patient Position |
Position the patient correctly (supine, lateral, or Trendelenburg), ensure comfort and adequate exposure. Adjust bed height and monitor angle for operator ergonomics.
Use positioning aids (pillows, wedges). Apply coupling gel adequately to eliminate air interfaces.

Identify standard anatomical landmarks and reference structures before attempting any measurement. Confirm orientation — head/feet, left/right.
Use vessels, organs, and fascial planes as fixed references. Verify sagittal, transverse, and coronal planes.

Acquire images in accepted standard planes. Optimize gain, depth, focus, and zoom so the structure fills the screen. Freeze the ideal frame before measuring.
One measurement per frozen frame. Use outer-to-outer or inner-to-inner as per protocol (e.g., liver length = outer-to-outer).

Uniform Caliper Placement |
Place calipers precisely on the edges of the structure following established measurement conventions. Zoom in if needed for accuracy; avoid estimating.
Leading-edge to leading-edge for wall thickness. Repeat 2–3 times and average for consistency.

Save annotated images with measurements displayed. Document all required views — at least two perpendicular planes for 3D structures.
Include patient ID, date, and side marker. Store in PACS with proper labeling and recorded values.
